Abstract

In his seminal work From Effectiveness to Efficiency: Random Reflections on Health Care, Archie Cochrane said, in 1971, 'More and more, requests for additional facilities will have to be based on detailed arguments with "hard evidence" as to the gain to be expected from the patient's angle and the cost. Few could possibly object to this.' These words are as true today as when they were written. Evidence-based clinical practice isolated from economic issues is not a realistic goal. Furthermore, narrow evidence-based medicine methodologies may lead to an inefficient health policy and greater inequalities in health. Evidence-based Health Economics recognises these issues and has brought together some of the world's leading thinkers and practitioners of economic evaluation in health care, all of whom have been involved with the "evidence-based movement", as participants, critics or both. These experts demosntrate not only the need for the use of evidence-based principles in economic evaluation but also that such principles should themselves be based on health economics. Case studies are outlined which show how to bring economic evaluation and systematic review together and how to use such evaluations in the clinical setting. The book also shows how economics can be used to broaden the evidence base for a more efficient and equitable health policy, and sets a future research agenda for this challenging area of work.

Abstract

Since its publication in 1987, Methods for the economic evaluation of health care programmes has become the standard textbook in this field world-wide Over the past ten years, the field of economic evaluation in health care has expanded considerably, with a rapid rise in the number of published studies, and wider recognition of their use in health care decision-making. Developments in economic evaluation have also led to the publication of several guidelines for study methodology, most recently those proposed by the United States Public Health Services Panel. The new edition of Methods for the economic evaluation of health care programmes follows the same basic structure as the first edition. The key methodological principles are outlined using a critical appraisal checklist that can be applied to any published study. The methodological features of the basic forms of analysis (cost analysis, cost-effectiveness analysis, cost-utility analysis, and cost-benefit analysis) are then explained in more detail. The book has been greatly revised and enlarged, especially with respect to cost-utility and cost- benefit analysis, where major methodological developments have taken place. New to this edition are chapters on collecting and analysing data, and presenting and using economic evaluation results. The new edition will be required reading for anyone commissioning, undertaking, or using economic evaluations in health care, and will be popular with health service professionals, health economists, and health-care decision makers. Abstract

The health systems we enjoy today, and expected medical advances in the future, will be difficult to finance from public resources without major reforms. Public health spending in OECD countries has grown rapidly over most of the last half century. These spending increases have contributed to important progress in population health: for example, life expectancy at birth has increased, rising on average by ten years since 1970. The challenge now is to sustain and enhance these achievements in a context of tight fiscal constraints in many countries combined with upward pressure on health spending from factors such as new technological advances and demographic changes. Finding policies that can make health spending more sustainable without compromising important achievements in access and quality requires effective co-operation between health and finance ministries. Sound governance and co-ordination mechanisms are therefore essential to ensure effective policy choices. Prepared by both public finance and health experts, this report provides a unique detailed overview of institutional frameworks for financing health care in OECD countries. One of the main features of this book is a comprehensive mapping of budgeting practices and governance structure in health across OECD countries.

Abstract

Patient organizations today play a major role in the scientific, economic and political arenas. They have become the quasi-obliged partners of researchers, industrialists and political authorities. This book investigates this quite recent configuration by offering an in-depth exploration of three thematic issues: To what social and political stakes are patient organizations confronted as regards the transformation, production, circulation, and governance of knowledge? The involvement of patient organizations in the economic world is acknowledged as confrontational, especially when it comes to relationships with the pharmaceutical industry. To what extent do opposition, "instrumentalization", or cooperation constitute relevant models if we are to account for the multifaceted relationships between patient organizations and economic actors? Patient organisations are the structuring actors of networks, coalitions, and collectives throughout Europe. What social and political concerns arise from these particular forms of collective action on a national or European scale? Each question is first examined through an academic and grey literature review. Then the emerging topics and critical issues are identified and discussed, drawing upon exchanges of experiences, viewpoints, and reflections between actors involved in patients' and users' movements and social scientists. The dynamics of patient organizations in Europe raises a number of questions of interest for all actors from the health system, as well as for social scientists. This book intends to contribute to the reflection on further research agenda setting and policy-making.
